Quick answer
Semaglutide (Ozempic, Wegovy) doesn’t cause classic withdrawal, but its effects wear off. In the STEP 1 trial extension, adults who stopped weekly semaglutide 2.4 mg after 68 weeks regained about two-thirds of their prior weight loss within a year (net loss fell from 17.3% to 5.6%), and most heart and metabolic improvements drifted back toward baseline. The NHS says to talk to your doctor if you want to stop, and not to stop suddenly; your doctor may suggest reducing gradually. If you take it for type 2 diabetes, your blood sugar may rise without it, so you’ll need a plan.

Symptoms, phase by phase
The NHS explains semaglutide works for weight loss by reducing appetite and helping you feel fuller, and for type 2 diabetes by lowering blood sugar. In the STEP 1 extension (327 participants followed for a year off treatment), people who had lost an average of 17.3% of their weight on semaglutide regained 11.6 percentage points by week 120, leaving a net loss of 5.6%. Lifestyle support was also stopped in that study. The Wegovy label says semaglutide has a half-life of about a week and stays in the blood for about 5 to 7 weeks after the last dose, so its effects fade gradually rather than all at once. Evidence note: this is one trial; results vary by person.
First weeks: Weeks 1 to 7 after the last dose
- Appetite and hunger gradually return
- Fullness after meals wears off
Months after stopping: Months 2 to 12

What makes withdrawal longer or shorter
- Why you take it. Diabetes control and weight management need different plans.
- Lifestyle support. In STEP 1, lifestyle intervention also stopped, which may have contributed to regain.
- Other diabetes medicines. Your doctor may need to adjust them.
When to see a doctor or get help now
- Before stopping (NHS).
- If you have diabetes and your blood sugar readings rise.
- If you’re stopping because of side effects, so they can be checked.
